Prizes & Awards

Please complete the forms listed below to receive your prize check. All documentation must be submitted either in person or via USPS mail to the Dean’s Office in 403 Avery Hall. Note that if your prize is a travel award, these documents must be submitted three months before you plan to travel to allow time for the award check to be issued.
Requirements for U.S. Citizens and Permanent Residents

Submit the following to the Dean’s Office (see Submitting Documentation instructions below):

  • Prize Request Form (see detailed instructions below)
  • Prize Agreement (Lowenfish Prize only)
  • Brief written request that includes tentative destinations and dates of travel (Kinne Prize only)
  • W-9 Tax Form – Complete your W-9 tax form using the same mailing address that you list on your Prize Request Form.
Requirements for Foreign Individuals

Submit the following to the Dean’s Office (see Submitting Documentation instructions below):

  • Prize Request Form (see detailed instructions below)
  • Prize Agreement (Lowenfish Prize only)
  • Brief written request that includes tentative destinations and dates of travel (Kinne Prize only)
  • W-8BEN Tax Form Please enter your foreign address under “permanent resident address” at the top of your W-8BEN tax form. If you have a U.S. address, you can include it under “mailing address.”
  • Visa Documents as applicable:
    a. Copy of passport identification page
    b. Visa / Visa Stamp in passport
    c. I-20 / DS-2019 / I-797 (Visa Certificate)
    d. I-94 (US Customs Arrival record)
Detailed Instructions
PRIZE REQUEST FORM
List the prize(s) you received as well as your contact information, including: long-term email address, phone number, and a deliverable USPS mailing address where you will receive mail at least three months after submitting your paperwork. Please note that the University does not accept PO Box addresses on any forms.
TAX FORMS
Please note that prizes may be taxed at varying rates depending on the IRS regulations/tax treaty specific to your country. Those taxes may be taken out of the prize check when it is issued. Please fill out the forms as best you can. If you have any questions, please check the IRS website. We are not allowed to provide tax advice of any kind.
PAYMENT DISTRIBUTION OPTIONS
If you are not able to receive and/or cash a check in USD at the mailing address confirmed on your tax form, then you may request a wire payment by checking the Wire Transfer box on the Prize Request Form. After you have submitted your prize award packet to the Dean’s Office, you will be contacted separately by the GSAPP Finance Office with further instructions on how to set up your wire transfer.
